Cornea Research Foundation of America (’‘CRFA’‘) is committed to the privacy and confidentiality of personal information. CRFA recognizes that the security of your personal information is of great concern to you. Our commitment to your privacy is reinforced by our adherence to the following website privacy policy.

Under NO circumstance will CRFA voluntarily disclose your personal or credit card information to any other party without your specific consent or as required by law.

USE AND DISCLOSURE POLICIES

The following paragraphs disclose CRFA’s information gathering and dissemination practices for this Web site.

Information collected on the Site comes in two forms:

  • Anonymous Information.  CRFA collects limited non-personally identifying information made available by your browser whenever you visit a website. Such non-personally identifying information includes your Internet Protocol (“IP”) address, your browser type, the time of your visit, the length of your visit, and the pages accessed during your visit. This non-personally identifying information is used to help us operate and improve the Site.
  • Personally Identifiable Information. Some areas on the Site allow you to contact CRFA to request product or other information or to make a monetary donation. In these areas on the Site, we ask you to provide us with certain personal information such as email address, street address, telephone, and credit card number, expiration date.

In order to provide you with accurate, useful and customized information, CRFA asks for the minimally necessary data which may include your name, address, telephone and email; and including your credit card information in order to complete your donation to CRFA. Your answers form the basis of the personal information that is unique to you.

Any transactions performed requiring your credit card information are performed in a secure environment, and credit card information is discarded immediately upon completing the necessary transaction(s).

Within the web site, you may be given the opportunity to indicate your preference to receive program information or participate in future research studies. If you indicate a preference to receive information, CRFA will use your contact information to send you information about CRFA, material about related products, or invitations to participate in research studies to assess the effectiveness of CRFA’s programs. 

Regardless of your response, you may opt-out of receiving future mailings by following the steps in the Choice/Opt-Out section below. 

CRFA will create aggregate data results for dissemination to sponsoring organizations and for research purposes. THIS AGGREGATE DATA WILL NOT CONTAIN ANY INFORMATION WHICH DISCLOSES YOUR IDENTITY. In this case, your data is combined with data of other participants to determine an overall profile of a population. These data become statistics and may be used by CRFA, or other organizations to understand the needs and characteristics for selected populations.

CRFA uses your IP address only in the event a Web server problem requires diagnosis and to administer the Web site and to count aggregate unique visits to the web site.

The CRFA site may have links to other sites that contain information CRFA believes may be helpful to you. CRFA cannot and will not be responsible for the privacy practices or the content of these other Web sites.

SECURITY MEASURES

This site has security measures in place to protect against the loss, misuse, and alteration of your information under CRFA’s control. These measures include: 
Authentication: CRFA uses eTapestry to provide authentication, trust, and SSL encryption. This allows site visitors to verify eTapestry with a trusted third party organization, while enabling communication with CRFA’s web sites using secure, encrypted channels.

Cookies: A cookie is a piece of data containing information about the user or user session. Usage of a cookie is not in any way linked to personally identifiable information while on the CRFA site. CRFA uses a session cookie to ensure that your session remains secure by maintaining an identifier unique to your session. The session cookie self-terminates after you close your browser. Persistent cookies are only used if you choose to have your login and password information maintained for future connections. The default setting is set to not store login and password information.

Encryption: CRFA employs SSL encryption in the use of its products. We recommend the use of a browser supporting 128-bit encryption though the site will support down to 40-bit encryption.  CRFA uses appropriate security measures to protect your personally identifiable information against unauthorized access, use or disclosure. Security measures include the use of firewalls and secure data storage facilities.

Choice/Opt-Out

The CRFA site provides users the opportunity to opt-out of receiving communications from CRFA programs and CRFA partners. Changes to the Policy

CRFA reserves the right to change this Privacy Policy from time to time. Any changes to this Privacy Policy will be immediately posted to this web page. If we make any substantial changes in the way we use your personally identifiable information, we will provide a prominent notice of the change on the Site.
